Connective tissue grafts (CTGs) are still considered to be the gold standard for these procedures; however, palatal harvesting is not always accepted by patients, time consuming and post-operative morbidity and limited availability are factors to be considered. The use of an acellular dermal matrix (ADM) in soft tissue peri-implant augmentation can provide a valid alternative to CTGs, making this procedure simple, fast and predictable, with excellent dimensional stability and aesthetic results.<\/p>\n Dimensional and structural alterations of the alveolar process are well-documented consequences of tooth extraction, with several factors influencing bone modelling, such as flapless extractions, ridge preservation techniques and immediate implant placement. Over the years, several biomaterials have been used for alveolar ridge preservation (ARP) procedures, however, it is extremely difficult to find an \u201cideal\u201d biomaterial suitable for all clinical scenarios. In this hands-on, several techniques will be described to guide the clinician in the decision-making process of biomaterial selection for ARP procedures.<\/p>\n Managing extraction sockets in the anterior maxilla is a challenge that dentists are facing on an everyday basis. Many techniques on how to counteract a possible resorption of the alveolar bone are known. While some of them require specific clinical skills, others may necessitate a longer period of treatment. Dr. Caiazzo\u2019s lecture will illustrate a simple procedure that may help to facilitate the management of these clinical case scenarios, with minimal interference to the natural healing time of a socket. He will also discuss a range of clinical situations, from socket preservation to horizontal and vertical augmentation, where the versatility of the botiss biomaterials portfolio can be a powerful tool to achieve aesthetic excellence.<\/p>\n There is an increasing need from patients for aesthetic fixed dental restorations with, or without implant placement. The alveolar jaw is a tooth dependent structure \u2013 any tooth extraction results in alveolar ridge resorption. Surgical techniques used today to preserve or augment deficient ridges follow biologic principles to achieve the same goal: a maintained or enlarged alveolar ridge with harmonious soft tissue contours. While the extent and pattern of resorption varies among individuals, there is an obvious loss of ridge contour as a result of physiological bone remodeling. The tendency for advanced resorption is even more increased around deep periodontal defects, especially when the buccal alveolar plate has been destroyed. Reconstruction of sufficient peri-implant hard- and soft tissue conditions is inevitable to achieve long term tissue stability and aesthetics. The efficacy of the applied surgical protocols to eliminate periodontal defects at adjacent teeth is also of high importance to achieve favourable soft- and hard tissue conditions and to level off peri-implant hard tissues to the proximal crestal bone of adjacent teeth. The course will present surgical approaches to reconstruct lost alveolar hard- and soft tissues applying socket preservation and guided bone regeneration techniques.<\/p>\n Developments in the microsurgery have led to an era of invisible dentistry. Technical improvements and advancement in biomaterials facilitate to augment the deficient sites. Despite these developments, the rules of the biology remain the same. Blood supply, handling of the soft tissues, flap thickness, design and tension are key elements to achieve optimal surgical outcome. These factors should be critically evaluated and controlled by clinicians. In this lecture, the requirements for periodontal\/peri-implant tissue reconstruction and health will be discussed in greater details.<\/p>\n
