INNOVATIVE APPROACHES IN SINUS GRAFTING PROCEDURES:
The fastest and most reliable reconstructions of severely atrophied posterior maxillae
The reconstruction of the atrophied posterior maxilla represents a daily challenge to most clinicians especially when they are dealing with severely pneumatized maxillary sinuses. The use of the lateral approach technique in such extreme cases is indicated when having a vertical residual bone height below 4 mm. This includes more invasiveness, higher morbidity to the patient and healing time between 6 to 9 months prior to the surgical re-entry and implant placement. This means practically dealing with a 12-month partially edentulous patient. For that reason, we propose a modification of the conventional lateral approach for sinus floor elevation to shorten the healing process period and reduce the patients’ morbidity by sparing them a second surgery being reserved for implant placement. More specifically, it’s possible to shortcut the staged technique by placing the implants simultaneously with the sinus grafting procedure, regardless of the minimal residual bone height. Here the implant loading can be implemented 3 months later, and the duration of the complete treatment will not exceed 4 months. The combination of sinus grafting procedure with horizontal bone augmentation by using autogenous bone block or sausage technique is also a very promising approach.
