Regeneration in a Stage III Grade C periodontitis case using maxgraft® +HyA (XS) and Emdogain®

Case presentation

A 35-year-old systemically healthy female patient with a positive smoking history presented after completion of non-surgical anti-infective periodontal therapy. Despite regular supportive care at 3-month intervals, several sites exhibited persistent deep probing depths. The patient was motivated toward smoking reduction and enrolled in surgical regenerative treatment. As part of the comprehensive therapy plan, all four third molars were extracted to eliminate additional periodontal risk factors.

Treatment

Following debridement and root surface conditioning, guided tissue regeneration was performed using maxgraft® +HyA XS in combination with Emdogain® (Straumann Group). The surgical approach aimed at the reconstruction of intrabony defects and the re-establishment of periodontal attachment.

Outcome

At 3-month re-evaluation, a marked reduction in probing depths and clinical inflammation was observed.

The 1-year follow-up demonstrated stable periodontal conditions with healthy soft tissue contours and no recurrence of pockets.

Aesthetic rehabilitation was completed by the referring general dentist through direct composite restorations, achieving further soft-tissue improvement by apical repositioning of the interproximal contact point (cf. Tarnow et al., 1992, J Periodontol 63: 995-1001).

Conclusion

This case illustrates successful regenerative therapy in a Stage III Grade C periodontitis patient using a hyaluronate-enriched allogenic bone substitute combined with enamel matrix derivative, achieving both functional stability and aesthetic integration after 12 months.
