A patient presented with a non-restorable maxillary right central incisor requiring extraction. Clinical and radiographic examination revealed a compromised labial bone plate, making preservation of the ridge contour essential for achieving an esthetic implant-supported restoration. The case was conducted as part of a clinical trial at the Faculty of Dentistry, The British University in Egypt, evaluating the application of NOVAMag® SHIELD in immediate implant placement procedures.
Following atraumatic tooth extraction using periotomes, care was taken to preserve the surrounding hard and soft tissue structures. An immediate implant was placed into the extraction socket.
To address the existing labial bone deficiency, a tunnelling technique was performed to create a subperiosteal pouch on the facial aspect while minimizing disruption of the periosteum and preserving soft tissue integrity. NOVAMag® SHIELD was then trimmed and adapted to the defect morphology. NOVAMag® SHIELD was positioned to support the facial contour, stabilize the augmentation site, and maintain the regenerative space.
Subsequently, cerabone®+HyA was placed into the gap between the implant surface and the socket walls to support bone regeneration and facilitate osseointegration. The combination of the cerabone®+HyA and the NOVAMag® SHIELD enabled effective contour augmentation of the facial defect.
To preserve the peri-implant soft tissue architecture and support esthetic healing, a provisional restoration was delivered. At follow-up, the provisional restoration was removed, revealing a well-maintained peri-implant soft-tissue contour and a naturally shaped emergence profile. The definitive restoration was subsequently delivered, demonstrating preservation of the facial contour and a harmonious esthetic integration with the surrounding tissues.
