Peri-implantitis is a biofilm-mediated inflammatory condition characterized by peri-implant mucosal inflammation and progressive bone loss, which may lead to implant failure if left untreated. Multiple risk factors contribute to its onset and progression. Keratinized mucosa plays an essential protective role against bacterial colonization and mechanical irritation; its absence, together with inadequate mucosal thickness, increases susceptibility to plaque accumulation, peri-implant inflammation, and mucosal recession, making it a key determinant in disease onset.

Effective prevention and treatment of peri-implantitis rely on the combined management of soft and hard peri-implant tissues. A reconstructive approach is proposed for cases presenting with peri-implant bone defects, aiming to achieve bone regeneration, possible re-osseointegration, and minimized mucosal recession. Selecting an appropriate bone graft material according to defect morphology is central to successful reconstruction.
