Hard and soft tissue augmentation in the aesthetic zone

Lateral and vertical bone defects are amongst the most challenging procedures in implantology in order to achieve satisfactory and safe regeneration. In addition to autogenous bone, which is still considered to be the gold standard for jaw augmentation procedures, various bone substitutes and membranes have shown promising results especially in lateral grafting applications. In contrast to grafts derived from patient´s own bone, the use of such biomaterials is not accompanied with donor site morbidity which leads to high patient acceptance. Bone substitute materials of different origin (bovine, synthetic or allogenic) have distinct biochemical properties, which are useful for indication oriented grafting procedures. However, soft tissue ingrowth and incomplete graft regeneration can be risks when using bone substitutes. Therefore, the use of barrier membranes is required. Of particular advantage are pericardium-based collagen membranes that can provide a long barrier function. Moreover, their native collagen structure, biocompatibility, tissue integration and nutrition transfer can significantly contribute to the graft stabilization. In addition, angio-selective membranes enable a fast transmembrane vascularization, which can be of crucial importance for the early stages of bone healing. The reliability of the final regeneration outcome is also dependent on the defect morphology, the quality of the recipient site, general health aspects of the patient, the selected healing time as well as the proper selection of biomaterials.
